PTO Pizza Party Lunch (Waiver REQUIRED)
Description
We want to take a moment to celebrate all of the hard work and dedication of our school community. The PTO will be providing FREE pizza on Friday, December 15, to all students. If you would like your child to receive pizza, this form must be completed by Friday, December 1.
Each student will receive 2-3 slices of pizza. Please feel free to still send your student with other lunch items they might enjoy along with their pizza (drink, chips, dessert). As always, there will be fresh fruit provided by the PTO.
*Thank you to all parents who completed the participation survey/waiver form. The deadline has passed and the order has been placed.
